Nope. Ohio is a direct access state you do not need a referral to get started with physiotherapy. This saves you valuable time and money, as well as avoids unnecessary diagnostic testing and medications. The sooner you receive treatment for an injury or dysfunction, the faster you will get well! After we complete your initial evaluation, we will write up a Plan of Care and send it to the provider of your choice, so they can be up to date on your treatments should you be interested in sharing that information.
At The Fusion Physio, LLC we are fee for service providers. We know that understanding insurance coverage can be complicated, that’s why we provide you with your cost for care upfront with full transparency. Clear is kind. That means no surprise bills in the mail months later! Yay!
If you should choose to self submit to your out-of-network benefits, we’ve got your back providing you with a superbill with all necessary documentation for reimbursement that is agreed upon by your insurance carrier. Here, we break it down further to simplify the process so you can focus on you instead of confusing paperwork!
Types of Insurance
HMO in-network benefits only, you will be responsible for 100% of cost.
PPO in-network and out-of-network benefits (this is usually on a percentage basis). For example you may be covered by your insurance 60% while you pay 40%.
Deductibles
The dollar amount you pay out-of-pocket prior to your insurance picking up some or all of the bill.
Some insurance companies have one deductible, other’s have both in-network and out-of-network deductibles.
Physical Therapy treatment at The Fusion Physio will be applied to your out-of-network deductible.
Copays
The dollar amount that is owed up front prior to care at an in-network provider. This payment goes towards your (in-network) deductible.
What is actually covered for me?
To find out details of your specific coverage, when calling your insurance company you will want to ask about your out of network physical therapy benefits. If you have an online portal you can also login here to check your benefits.
For your information, our therapists apply only covered diagnosis (ICD-10) codes and include service or procedural (CPT) codes provided at the time of service.
What’s the process?
Once you have been provided physical therapy services, all client specific coding and necessary personal information will be provided in a superbill so that you may self submit to your insurance for reimbursement purposes. This will be sent to you via email after payment has been rendered.
